Please use this form for the following reasons:

To claim an Expense Reimbursement:
where you have use your personal funds to purchase items for the school / school related activity.
1. Indicate the Department / Cost Centre which will pay for this reimbursement. NOTE: No other reimbursement from any other source, can be claimed with respect to this claim.
2. Please attach the following:
* GST receipt(s)
* a signed Purchase Order form (PINKIE) from the Budget holder for each reimbursement claim

To claim Mileage:
Where you have had to use your personal vehicle (when a school van / transport is not available) for a school related Activity / event, or in the case of a camp, are the dedicated emergency vehicle or requested by the Activity organiser. Any claim for travel outside metropolitan Auckland needs prior approval from the Principal
1. Indicate the Department / Cost Centre which will pay the mileage
2. List each trip separately, including a start and end point
3. Indicate accurate mileage (use Google Maps to assist)

For an EOTC Reimbursement:
Staff are entitled to a Field Allowance of $25 for each NIGHT and there after, per day or part day (min 10 hours). This is when a teacher is supervising students attending a school camp or school trip for more than one complete day - as per Secondary Teachers’ Collective Agreement.
